Job Description

Overview We are seeking a dedicated and highly skilled Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings including long-term care, assisted living, hospitals, and outpatient services. As an LVN, you will play a vital role in providing compassionate, comprehensive nursing care to diverse patient populations, ensuring safety, comfort, and optimal health outcomes. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and health histories to determine individual care needs. Administer medications and injections accurately following physician orders while adhering to safety protocols and HIPAA regulations. Assist with invasive procedures such as IV infusion, catheterization, and wound suturing using aseptic techniques. Monitor patients' conditions continuously, recognizing signs of deterioration or acute pain requiring immediate intervention. Document all patient interactions and treatments precisely within electronic health record (EHR) systems such as Epic or Cerner for accurate medical records management. Collaborate with interdisciplinary teams—including physicians, therapists, and case managers—to develop and implement personalized care plans. Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or those requiring hospice or palliative support. Support discharge planning by educating patients and families on medication management, post-care instructions, and community resources. Qualifications Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience in medical-surgical units, emergency rooms, ICU, or specialized settings such as long-term care or assisted living facilities. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for documentation and patient tracking. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to diverse clinical scenarios. Experience in IV infusion therapy, medication administration, phlebotomy, and sterile procedures such as suturing or specimen collection. Excellent communication skills with the ability to triage effectively and work collaboratively within multidisciplinary teams. Ability to adapt quickly in high-pressure environments such as trauma centers or acute care units; prior experience in Level I/II trauma centers is advantageous.
