Job Overview Ukiah Post Acute is a beautiful 52-bed facility in the heart of Ukiah, CA. We are seeking a compassionate and dedicated Licensed Nurse (LVN on RN) to join our healthcare team on the Night Shift. The ideal candidate will be responsible for providing high-quality nursing care to our short-term rehab and long-term patients and will work closely with other healthcare professionals to ensure optimal patient outcomes.
Needs:
Part-time or Full-time NIGHT (10:00 PM - 6:30 AM) shifts available.
Pay:
Subject to licensure and dependent on experience. Responsibilities Administer medications and treatments as prescribed by physicians. Monitor and record vital signs, ensuring any changes in patient condition are reported promptly. Conduct diagnostic evaluations to assess patient health status. Provide primary care services, including wound care and catheter management. Maintain accurate medical documentation for all patient interactions and treatments. Collaborate with healthcare teams for effective case management and patient care planning. Educate patients and their families on health maintenance and disease prevention. Qualifications Valid Licensed Nurse (LVN or RN) license in California. Experience in skilled nursing is preferred. Excellent communication skills, both verbal and written.
