Ventura, CA Job Details Travel nursing | PRN | Per diem $50 - $58 an hour 16 hours ago Benefits Professional development assistance Flexible schedule Qualifications Teamwork Vital signs Triage HIPAA Home health Full Job Description Overview We are a growing, physician‑led mobile wound care practice delivering advanced, in‑home wound management to patients throughout Ventura and Santa Barbara Counties. Our clinical focus includes advanced wound healing modalities , such as non‑contact, low‑frequency ultrasonic therapy, and evidence‑based treatment plans tailored to each patient's needs. As a small, collaborative team, we prioritize clinical quality, efficiency, and compassionate patient care , ensuring patients receive specialized wound care directly where they live. Join our dedicated healthcare team and play a key role in expanding access to innovative, patient‑centered wound care in the home setting. Conduct comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. Develop and implement individualized care plans; coordinate discharge planning and case management activities to ensure seamless transitions of care. Conduct comprehensive in‑home patient assessments , including vital signs, focused physical exams, wound evaluations, and detailed health history documentation using EMR/EHR systems. Administer wound‑related therapies (Ultramist)safely and accurately, following HIPAA guidelines and established clinical protocols; wound care/wound vac advanced wound modalities. Support the development and implementation of individualized wound care plans; assist with care coordination, discharge planning, and transitions of care to ensure continuity between home health, primary care, and specialty providers. Maintain meticulous medical documentation, including accurate CPT/ICD‑10 coding for wound procedures, debridement, advanced therapies, and other clinical services performed in the home. Collaborate with multidisciplinary teams—including physicians, NPs/PAs, home health nurses, therapists, and case managers—to support urgent needs, triage concerns, and deliver high‑quality post‑acute wound care.
Pay:
$50.00 - $58.00 per hour
