We pride ourselves on fostering a compassionate and professional environment where patient well-being is our top priority. We are seeking a motivated Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. In this vital role, you will provide high-quality nursing care, support patient recovery, and ensure comfort for residents across various care settings. Your expertise will help us maintain our reputation for excellence in post-acute and long-term care. Administer medications, including injections and IV , following physician orders and safety protocols Conduct patient assessments, vital sign monitoring, and physical examinations to evaluate health status Assist with complex procedures such as catheterization, wound care, and sterile techniques Manage patient care plans, including discharge planning and documentation in electronic health record (EHR) systems like PCC Provide specialized care for residents with dementia, Alzheimer's disease, or other memory impairments, including memory and behavioral health support Support acute pain management, hospice care, and infection control measures to promote optimal recovery and comfort Collaborate with interdisciplinary teams on case management, triage, and emergency response efforts in high-pressure environments such as ICU or emergency rooms Valid LVN license in the state of practice with current certification Familiarity with EMR/EHR systems including PCC for accurate medical documentation Experience working with diverse populations including geriatrics, individuals with disabilities, or those in assisted living or home care environments Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and infection control standards Strong assessment skills Excellent communication skills to effectively coordinate with healthcare teams and educate patients on health coaching and medication administration Join us at Spring Valley Post Acute where your skills will make a meaningful difference every day!
