Job Description

Job Overview We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our dynamic home health/ healthcare team. In this vital role, you will provide compassionate patient care in a home health setting. Your expertise will help ensure optimal health outcomes by delivering skilled nursing services, performing assessments, administering medications, and supporting patients with complex medical needs in the comfort of their own home. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations while working in a fast-paced, collaborative environment. Responsibilities Perform comprehensive patient assessments, vital signs monitoring, and physical examinations to evaluate health status. Administer medications and injections accurately following physician orders, including IV infusions and other specialized treatments. Manage complex cases involving acute pain, dementia care, Alzheimer's care, and memory care with sensitivity and professionalism. Operate electronic health record (EHR) systems such as well sky kinnser, Epic, Cerner, or Athenahealth to document patient information meticulously. Assist with diagnostic evaluations including specimen collection/processing, medical imaging coordination, and laboratory testing. Provide triage services by assessing patient symptoms and determining appropriate levels of care in urgent or emergency situations. Support discharge planning, case management, and patient education to promote recovery and well-being. Maintain infection control protocols and aseptic techniques to ensure patient safety during procedures like catheterization or sterile processing. Experience Valid LVN license with current state licensure required; additional certifications in Basic Life Support (BLS) strongly preferred. Prior experience working in diverse healthcare environments such as hospitals (including ICU, ER), nursing homes, hospice care, or outpatient clinics is highly desirable. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate documentation. Experience working with individuals with disabilities or developmental delays enhances your ability to provide personalized patient-centered care. Knowledge of pharmacology related to pain management, infectious disease care, and acute care procedures such as tube feeding or ventilator management is advantageous. Strong assessment skills in physiology and anatomy knowledge combined with effective communication for patient education and triage support. $45.00 - $50.00 per hour
