Licensed Vocational Nurse (LVN) - PRN/Part-Time Join the Clinical Team at Rossmoor Post Acute Rossmoor Post Acute is seeking a compassionate, dependable, and clinically skilled Licensed Vocational Nurse (LVN) to join our dedicated nursing team on a PRN/Part-Time basis. Located in the Rossmoor community of Walnut Creek, we are committed to providing high-quality skilled nursing and rehabilitation services in a supportive, resident-centered environment. This is an excellent opportunity for an LVN seeking a flexible schedule while maintaining consistent part-time hours. Position Details

            • Provide compassionate, high-quality nursing care to residents in accordance with individualized care plans.
            • Administer medications and treatments as ordered and in accordance with facility policies.
            • Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.
            • Perform nursing assessments and document resident care accurately and timely.
            • Assist with wound care, treatments, and other nursing procedures as directed.
            • Monitor vital signs and other clinical indicators as appropriate.
            • Communicate effectively with residents, families, physicians, and members of the interdisciplinary care team.
            • Assist with admissions, transfers, and discharges as needed.
            • Maintain accurate and complete nursing documentation.
            • Follow all applicable federal, state, and facility regulations, policies, and procedures.
            • Promote resident safety, dignity, independence, and quality of life. Qualifications
            • Current and active California LVN license in good standing.
            • Current CPR certification preferred/required per facility policy.
            • Previous skilled nursing or post-acute care experience preferred.
            • Strong clinical assessment, communication, and organizational skills.
            • Compassionate, dependable, and resident-focused.
            • Ability to work effectively as part of an interdisciplinary healthcare team.
            • Must be able to commit to 24-32 hours per week.
