  • Severe and Persistent Mental Illness (SPMI)Position Summary The Licensed Practical Nurse (LPN) provides direct nursing support and coordination of medical care within a residential treatment environment serving adults with Severe and Persistent Mental Illness (SPMI).
This role is responsible for medication management, medical triage, coordination of care, chronic disease support, documentation, and collaboration with psychiatric and primary care providers to ensure safe, effective, and integrated client care. The LPN plays a critical role in supporting clients with complex psychiatric, medical, and co-occurring conditions by promoting continuity of care, medication compliance, client wellness, and timely access to appropriate medical services. Essential Duties and ResponsibilitiesMedication Management & Nursing Support Conduct routine medication audits to ensure accuracy, compliance, and safety. Maintain and update Medication Administration Records (MARs). Order, track, and reconcile medications and medication deliveries. Coordinate medication refills with pharmacies and prescribing providers. Monitor clients for medication effectiveness, side effects, adverse reactions, and medication compliance. Assist in managing psychotropic medications and medications related to chronic medical co-morbidities commonly associated with long-term SPMI populations. Communicate medication concerns, changes, and observations to psychiatric and medical providers. Medical Coordination & Triage Assess and triage client medical concerns and determine appropriate levels of care. Coordinate referrals to primary care providers, specialists, urgent care, emergency rooms, and hospitals as clinically indicated. Respond appropriately to acute medical concerns and emergencies. Support continuity of care following emergency room visits, hospitalizations, and specialty appointments. Monitor and support clients with chronic medical conditions such as diabetes, hypertension, COPD, obesity, cardiovascular disease, infectious diseases, and other co-occurring health conditions. Documentation & Compliance Complete timely, accurate, and clinically appropriate documentation in the electronic medical record (EMR). Maintain documentation related to medication administration, nursing assessments, provider communications, incident reports, and client health status. Ensure compliance with all federal, state, licensing, and accreditation standards. Support medication storage, medication room organization, and regulatory readiness. Interdisciplinary Collaboration Collaborate closely with psychiatrists, primary care providers, therapists, case managers, and residential staff. Support on-site psychiatric and primary care services through preparation, communication, and follow-up coordination. Participate in interdisciplinary treatment team meetings and contribute nursing observations and recommendations. Educate clients and staff regarding medications, health conditions, wellness practices, and treatment recommendations. Client Care & Support Promote a therapeutic, trauma-informed, and recovery-oriented treatment environment. Support clients in improving health literacy, medication adherence, and self-management skills. Assist clients in accessing preventative healthcare services and maintaining continuity of medical care. Maintain professional boundaries and provide compassionate care to individuals with complex psychiatric and medical needs. Qualifications Current and active Licensed Practical Nurse (LPN) license in the State of Colorado (or applicable state). Minimum of one (1) year nursing experience preferred. Experience working with psychiatric, behavioral health, substance use, or residential treatment populations strongly preferred. Knowledge of psychotropic medications, chronic medical conditions, and co-occurring disorders preferred. Ability to assess and respond appropriately to medical and psychiatric concerns. Strong organizational, communication, and documentation skills. Ability to work collaboratively within a multidisciplinary team environment. CPR/First Aid certification required or ability to obtain upon hire. Preferred Skills Experience with Electronic Medical Records (EMR/EHR). Familiarity with residential treatment operations and behavioral health regulations. Strong crisis management and de-escalation skills. Ability to manage multiple priorities in a fast-paced treatment environment. Understanding of trauma-informed care and recovery-oriented treatment approaches. Physical Requirements Ability to sit, stand, walk, bend, and lift up to 25 pounds as needed. Ability to respond to medical or psychiatric emergencies within the facility. Ability to work in a residential treatment setting with individuals experiencing significant psychiatric symptoms. Work Environment This position operates within a residential behavioral health treatment setting serving adults with Severe and Persistent Mental Illness (SPMI). The environment may include exposure to clients experiencing psychiatric crises, emotional distress, or acute medical needs. Professionalism, flexibility, compassion, and strong clinical judgment are essential to success in this role. Position Purpose The Licensed Practical Nurse supports the mission of the organization by helping ensure that clients receive safe, coordinated, and compassionate medical and psychiatric care while promoting recovery, stability, wellness, and improved quality of life.
