Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately while adhering to safety protocols and documentation standards. Assist with complex procedures such as IV infusions, catheterizations, wound suturing, and sterile technique practices. Manage electronic health records (EHR/EHR systems) efficiently to ensure precise medical documentation and compliance with HIPAA regulations. Provide specialized care for patients with Alzheimer's disease, dementia, or memory impairments in memory or hospice care settings. Collaborate with interdisciplinary teams to develop and implement personalized care plans for patients with acute or chronic conditions. Educate patients and their families on health management strategies, medication use, and post-discharge instructions to promote recovery and independence. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or region. Proven experience working in diverse healthcare environments such as long-term care facilities, hospitals (including ICU or trauma centers), assisted living communities, or home care settings. Strong knowledge of medical terminology, anatomy, physiology, and clinical procedures including IV insertion and catheter care. Familiarity with EMR/EHR systems like Cerner or Athenahealth for accurate medical documentation. Experience working with geriatric populations or individuals with disabilities; knowledge of dementia and Alzheimer's care is highly desirable. Ability to perform physical examinations, vital sign assessments, specimen collection/processing, and basic life support (BLS). Excellent communication skills for caregiver interaction, patient education, and team collaboration; sensitivity to patient confidentiality policies such as HIPAA is essential.
