$38.00 - $47.00 per hour, depending on experience, plus shift differentials

+$1.00/hr 11 PM-7
AM:
+$1.00/hr Weekends (All Shifts): +$1.50/hr
POSITION OVERVIEW
Mystic Healthcare & Rehabilitation Center is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our nursing team. In this role, you will deliver direct patient care, administer medications, and collaborate with RNs and interdisciplinary staff to ensure the highest quality of care for our residents in a long-term care and rehabilitation setting.
KEY RESPONSIBILITIES
Administer medications and treatments as prescribed and in accordance with facility policies Monitor and assess residents for changes in condition; report findings to the RN or attending physician Perform wound care, catheter care, and other skilled nursing procedures Take and document vital signs, intake and output, and other clinical data Assist with admissions, discharges, and care plan development Communicate effectively with residents, families, physicians, and interdisciplinary team members Supervise and delegate tasks to CNAs and other support staff Respond to emergencies and provide first-line intervention as appropriate Maintain accurate and timely documentation in accordance with state and federal regulations Follow all infection control, safety, and HIPAA compliance standards
QUALIFICATIONS & REQUIREMENTS
Active Connecticut LPN license in good standing (required) High school diploma or GED required; completion of an accredited LPN program required Long-term care or skilled nursing facility experience preferred CPR/BLS certification (or willingness to obtain upon hire) Strong clinical assessment and critical thinking skills Ability to work independently and as part of a collaborative team Excellent communication and organizational skills Ability to stand, walk, and lift up to 50 lbs for extended periods
