Job Description

LPN - Home Health We are seeking a compassionate and reliable LPN to join our growing home health team. In this role, you will provide skilled nursing care to patients in their homes, helping them maintain their health, independence, and quality of life.
Responsibilities:
Provide direct patient care in accordance with the care plan. Monitor and document patient health status, symptoms, and progress. Administer medications and treatments as prescribed. Assist with wound care and other nursing procedures within scope of practice. Educate patients and family members on health management and treatment plans. Communicate changes in patient condition to the supervising RN and healthcare team. Maintain accurate and timely clinical documentation. Ensure compliance with all agency policies and applicable regulations.
Qualifications:
Current LPN license in the state of practice. Previous home health experience preferred but not required. Strong communication and interpersonal skills. Ability to work independently and manage time effectively. Reliable transportation and valid driver's license. Current CPR certification preferred.
