PER DIEM/PRN LPN
to join our team in Norwalk, CT! Our Licensed Practical Nurses (LPNs) are responsible for providing care for our clients and patients in their home or community setting. The Licensed Practical Nurse
  • LPN is responsible for assisting in the planning for the patient's and family care under the direction of the RN the supervision of a registered nurse.
Responsibilities include following the plan of care established by physician, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. The Licensed Practical Nurse supports the philosophy, goals and culture of Constellation Health Services.
Responsibilities:
Provides nursing care as defined in State Nurse Practice Act, including specialized duties related to end of life care. Implements current nursing practice following a comprehensive assessment and the plan of care. Maintains accurate and timely documentation of patient services to reflect the plan of care. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care. Recognize and interpret symptoms and institute remedial measures within the scope and limits of a Licensed Practical Nurse. Observes records and reports to immediate supervisor if any changes in patient's condition. Participates in coordination of hospice and other home care services, appropriately reporting the identified needs to the interdisciplinary group. Examples include RN, hospice aide, MSW, OT, PT, ST, Registered Dietician, Spiritual Counselor or Clinical Supervisor. Participates in Hospice IDT meetings Implements care to achieve outcomes of comfortable and safe dying and effective grieving for the patient and family. Uses and prepares equipment and supplies effectively, adhering to aseptic technique. Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications. Participates in personal, professional growth and development. Also, participates in organization's quality assessment performance improvement team activities. Performs other duties as assigned by the registered nurse or clinical supervisor. Some weekend days are required, and summer and winter holiday commitments as needed

Requirements:
Graduate of an accredited practical nurse program. Licensed LPN in the state of employment At least 1-2 years of LPN experience is a MUST ; Home care preferred Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus Have reliable transportation and be willing to commute to provide the best in-home patient care. Alternating weekend days are required, and summer and winter holiday commitments as needed. Basic knowledge of iOs operating system
