Licensed Practical Nurse The Cottage at Litchfield Hills

THIS POSITIONS STARTING PAY IS
$32.00 PERH PART TIME & PER DIEM POSITIONS ONLY!
Responsibilities:
- Provide direct resident care, including administering medications, performing diagnostic evaluations, and providing treatments as prescribed by physicians
- Collaborate with the healthcare team to develop and implement individualized care plans for patients
- Monitor and assess residents conditions, including vital signs, symptoms, and changes in health status
- Perform assessments of residents
- Document resident information, treatment plans, and progress notes accurately and timely using electronic medical record systems
- Educate patients and their families on disease management, medication administration, and home care instructions
- Coordinate and communicate with other healthcare professionals to ensure continuity of care for patients transitioning between hospital, home care, or other healthcare settings
- Provide compassionate care for senior patients in long-term care facilities or assisted living settings
Qualifications:
- Valid Licensed Practical Nurse (LPN) license in CT
- Minimum of 2 years of nursing experience in an assisted living setting
- Proficiency in performing physical examinations, administering medications, and providing direct patient care
- Strong knowledge of medical terminology, disease processes, and treatment modalities
- Experience with electronic medical record systems for documentation purposes
- Excellent communication skills to effectively interact with patients, families, and the healthcare team
- Ability to prioritize tasks and manage time effectively in a fast-paced environment
- Compassionate attitude towards patients of all ages and backgrounds
