Shift Detail:
8-4:30 Mon-Fri. Cover Torrington and surrounding towns. Cover 1 weekend a month, 1 major and 2 minor holidays a year. Work where every moment matters. As part of a person-centered care model, and under the direction of the RN, this role will provide skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care. This role will collaborate with all team members to achieve exceptional outcomes. Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN. Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability. Records daily progress in EMR. Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases. Communicates/conferences with primary nurse and other disciplines. Consults with physician regarding client needs and plan of care. Assists in evaluation of care given by paraprofessional staff. Participates on agency committees and special projects.
Qualifications Minimum Requirements:
  • High School diploma or GED
  • Successful completion of a CT state approved LPN certificate program Experience
  • Computer skills including but not limited to Outlook, Microsoft Word, and Excel Licensure
  • Current and valid LPN license
