Job Description

Alvita Care is seeking experienced Licensed Practical Nurse LPNs whose first language is English and who also speak an additional language (Bilingual). This is per diem role for Western Connecticut (Fairfield, New Haven, Litchfield County) Please make sure to include a resume highlighting your healthcare experience, credentials, availability, and all additional languages you speak , including your level of proficiency. DETAILS:
Flexible Hours! Shifts from 6hrs, 8hrs to 12hrs . Day and Night Shifts Available Work with us part-time, per diem or around your current work schedule Shifts are assigned to provide 1:1 care in the home environment
QUALIFICATIONS
CT LPN licensure. BLS, ACLS, or CPR certification. Minimum of 6months to (1) year demonstrated experience as an LPN Ability to put patients at ease and explain physician's instructions. Ability to multi-task with excellent organization skills. Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds. Knowledge of medical terminology. Team player and ability to work independently Valid driver's license or Non-Driver photo ID card Drivers preferred!
ESSENTIAL DUTIES & RESPONSIBILITIES
Assessments needs of the clients to develop a plan of care Consult with Management on complex/unique cases Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients/patients/. Monitor/Assess patients/clients' condition, effects of the care plan, Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals. Manages, plans, coordinates and executes the patient's safe discharge from service. Ensures that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements. Communicates and consults with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload. Assists patients with activities of daily living Documents content and process of all services provided in internal system properly. Transport/utilize medical equipment to and from clients/patients' home/facilities. Travel to patient/clients' homes and/or other facilities. Perform other related duties and participates in special project, as assigned. We have private-duty one-on-one care cases available as well as facility based care cases. All applicants are subject to a phone screening to determine compatibility and overall availability of accommodations for both the caregiver and the agency. Please note that cases with Alvita Care are only available to caregivers who successfully complete the onboarding process.
