7a-3p - Rotating Weekends. Job Summary We are seeking a dedicated and highly skilled Licensed Practical Nurse (LPN) to provide compassionate, comprehensive patient care. The ideal candidate will possess a strong foundation in nursing practices, with experience in long-term care. As an LPN, you will play a vital role in supporting patient well-being through direct care, medication administration, and health assessments while adhering to strict safety and confidentiality standards. This position offers an opportunity to make a meaningful difference in patients' lives by delivering high-quality nursing services with professionalism and empathy. Duties Perform detailed patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems. Provide wound care, including dressing changes and suturing support, ensuring aseptic technique at all times. Collaborate with multidisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices. Manage emergency situations effectively through first aid skills, airway management, and CPR certification while maintaining clinical confidentiality policies. Utilize EMR/EHR systems for accurate medical documentation, medication administration records, and client communication. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) from an accredited licensing board. Proven experience working in diverse healthcare environments such as nursing homes, assisted living facilities, or home & community care settings. Strong communication skills with the ability to effectively interact with patients of all ages—including geriatric populations—and their families. Certifications in Basic Life Support (BLS) are required.
