Job Description

Overview:
Provide one-on-one nursing care and treatment to patients in their homes. Use a comprehensive and patient
  • specific care plan with innovative solutions.
Spend quality time with the patient, in their home, to promote their health and independence. Collaborate with the Clinical Director, patients and their families and other members of the patients care team to implement and follow a patient specific care and treatment plan. Cooperatively utilize varied solutions to educate and promote the health and independence of the patient. Assess, monitor, document and report progress of patients' health and condition using documents such as clinical progress notes within established timelines.
Responsibilities:
  • Provide nursing care in a home setting
  • Complete nursing assessments initial and reassessments in home healthcare settings
  • Specialize in dementia care for patients with memory impairments
  • Specialize in pediatric care for patients in home healthcare settings
  • Utilize knowledge of ICD-9 and ICD-10 coding for accurate documentation
  • Apply physiology knowledge to assess and monitor patient conditions
  • Collaborate constantly with Clinical Director for patient care coordination
Experience:
  • Must be a Licensed Practical Nurse (LPN) with current state licensure
  • Minimum of 2 years of experience in a Home Healthcare setting
  • Strong understanding of medical terminology and procedures
  • Excellent communication and interpersonal skills to interact with patients, families, and healthcare team members efficiently This position offers an exciting opportunity for an LPN to utilize their skills in various home healthcare settings while providing compassionate care to patients.
