CenterWell Home Health seeks a compassionate Home Health Licensed Practical Nurse to deliver skilled, in-home nursing care. The LPN will perform assessments, administer medications and treatments, manage wounds, and monitor chronic conditions while following individualized care plans. You will educate patients and families, promote safety and independence, and document care accurately in EMR systems. Working closely with an interdisciplinary team, you'll support high-quality outcomes and patient satisfaction. Responsibilities Provide in-home skilled nursing care, including assessments, treatments, and patient monitoring Administer medications and treatments per provider orders and agency protocols Document all care accurately in EMR and communicate changes in condition to the RN and physician Educate patients and caregivers on disease management, medications, and home safety Collaborate with an interdisciplinary team to support individualized care plans and quality outcomes Adhere to infection control, safety, and regulatory standards while traveling between patient homes Required Skills Wound care and dressing changes Medication administration (oral, injection, IV under RN/MD direction) Vital signs monitoring and assessment Care plan implementation and documentation (EMR) Patient and family education Catheter and ostomy care Home health regulations and documentation standards Infection control and safety practices IV therapy and phlebotomy basics Time management and route planning
