Job Description
Job Title:
Home Care Coordinator Job Summary Responsible for the development and implementation of homecare services for Florida PACE Center participants, including the coordination of all Durable Medical Equipment and nursing home care. Essential Job Functions- Assesses the homecare needs of participants, and identifies and develops specific plans of care
- Assist in Safety evaluations for Prospect prior to enrollment with documentation and uploading in EMR
- Coordinates the implementation of all homecare services which includes nursing, physical therapy, occupational therapy, speech therapy and personal care services to ensure that quality services is provided to meet client needs
- Coordinates and authorizes all Durable Medical Equipment, Consumables, and Frozen Meals
- Coordinates and supervises all contracted nursing care for FPC participants in the nursing home/boarding home according to program standards. Includes interaction with other facility staff
- Assesses program homecare services system, makes recommendation and offers solutions to revise or modify the system to better meet participant's needs
- Actively participates in Multidisciplinary Team Meetings
- Performs compliance monitoring of contracted home care agencies and personnel. This includes all ALF audits for Quality purposes
- Participates in department meetings and in-services as necessary
- Prepares and Coordinates Health Assessment Form 1823 for ALF's Contracted by Florida Pace.
Job Requirements Education:
- Graduate of an accredited school of practical nursing preferred or higher level of education
Experience:
- One to three years of experience working with frail elderly population and home care (or a combination of experience and education)
Licenses/Certifications:
- Current LPN license by the Florida State Board of Nursing, preferred
- Current BLS Certification
- Valid Florida Driver's License and Proof of Insurance Abilities Required
- Sensitivity when working with impaired elderly clients is required
- Must have good interpersonal skills and ability to work with staff, clients and providers of diverse ethnic background
- Must be organized and demonstrate good judgment
- Must have good communication skills in English
- Knowledge of Spanish may be required
- Must have Florida Driver's License, insurance and have access to automobile for field visits
- Must be able to demonstrate good judgment regarding personal and client safety in their role as a community service provider
- Must be willing to conduct in-home visits anywhere in Dade and Broward counties
Functional Demands Environment Work Conditions:
- Conducts home visits, enters into private home of FPC enrollees, frequently in unsafe neighborhoods
Infectious Material Exposure:
- May experience exposure to a variety of infectious material/illnesses in home/community settings
- Must be able to recognize potential or actual hazards and deal with them in an appropriate manner Organizational Expectations
