Looking for a flexible weekend role where you can truly make an impact? Join our home health team and provide one-on-one care to patients in the comfort of their homes.

PRN Licensed Practical Nurse (LPN/LVN)Location:
Polk County Schedule:
Weekends (as needed)What You'll Do:
Provide skilled nursing care in the home setting under the direction of an RN Follow physician orders and individualized plan of care Monitor patient conditions and report changes to the RN and physician Administer treatments and perform nursing procedures per agency standards Educate patients and caregivers on medications, disease processes, and home care Document visits in compliance with Medicare guidelines Ensure adherence to HIPAA, infection control, and regulatory requirementsQualifications/Educational Requirements:
Licensed Practical Nurse (LPN) from a state-approved, accredited program. Oasis Home Health Care 1 year of experience. Minimum 2 years of healthcare experience Strong communication and organizational skills Ability to work independently and manage time effectively Understanding of Medicare/Medicaid regulations (preferred) Reliable transportation for travel to patient homes Must haveAHCA II Background Check:

Follow-up assessment and documentation of observed patient's condition and interventions provided at each visit 2. Complete patient evaluation tasks, including medication administration and vital signs 3. Administer Physician-prescribed medications 4. Wound care and maintenance of skin integrity 5. Reinforce patient and family education on appropriate home health care procedures and strategies 6.
