Job Description

About our company: Health Care of South Florida Corp is a CHAP accredited home health agency established in 2005 servicing Miami-Dade, Broward, and West Palm counties. We specialize in a wide range of services including skilled nursing, physical therapy, and non-skilled home health aide care, ensuring exceptional healthcare delivery to our diverse community.
Responsibilities:
Provide skilled nursing care including wound care and insulin injections. Deliver care to patients in both facility settings and home environments. Monitor patient conditions and report any changes to supervising RNs or physicians. Assist in implementing and following individualized care plans. Ensure compliance with HIPAA regulations and agency protocols. Identify and anticipate patient care needs to enhance quality of care. Maintain accurate and timely documentation of all patient interactions and treatments.
Qualifications:
Active LPN license in the state of Florida. Bilingual (Required). Experience in Home Health (Must). Experience with wound care and insulin administration. IV Certification (Plus). Knowledge of Kinnser (Plus). Current CPR certification (Must) Must have all credentials in place, including Level II Background Screening and recent in-services completed. Strong interpersonal skills with a personable and proactive approach to patient care.

From $25.00 per hour
