Responsibilities:
Provide direct patient care in home settings in accordance with physician-directed care plans Monitor and document patient health status, vital signs, and progress notes Administer medications, wound care, and other skilled nursing treatments Communicate effectively with patients, families, and the care team regarding patient conditions and changes Educate patients and caregivers on health management, medications, and safety Maintain accurate and timely clinical documentation in compliance with state and agency standards Collaborate with RNs, therapists, and other healthcare professionals to ensure continuity of care
Requirements:
Active and unrestricted LPN license in the state of Florida Previous home health or clinical nursing experience preferred Strong knowledge of skilled nursing procedures, medication administration, and wound care Excellent communication, documentation, and time management skills Reliable transportation and valid driver's license required CPR/BLS certification required or ability to obtain upon hire Compassionate, patient-centered approach with a strong sense of professionalism
