LPN Home Health (PRN)

Empath Home Health , a member of Empath Health is seeking a compassionate PRN Home Health LPN to work with our Home Health team to deliver individualized care to patients in their homes in the Port Charlotte/Venice area.

Locations:

Home health visits in Charlotte county (including Port Charlotte, North Port, Rotonda West, Venice, Englewood, Arcadia, north Punta Gorda , and surrounding areas)

Schedule :

PRN role; 10-15+ home health visits per week What you'll Do Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient's plan of care under the direction and supervision of a Registered Nurse. Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator. Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures. Perform patient and family instruction on diet, medication, and treatment. Minimum of 1-2 years of experience as a LPN. Home health care experience preferred. Strongly prefers experience using Homecare Homebase (HCHB) EMR Intravenous (IV) Certification strongly preferred.
