Stuart, FL Job Details Part-time | Full-time | Per diem 6 hours ago Qualifications CPR Certification OSHA Patient assessment BLS Certification English Driver's License Nurse Licensure Compact (NLC) Multistate LPN License Full Job Description The LPN provides assigned nursing care under the direction of a Registered Nurse (Case Manager) who is accountable to the Nursing Supervisor / DON.OSHA defines this position as Class 1 exposure to blood.
Functions:
Prepares progress notes for the clinical records. Reports any changes in the patient's conditions to his/her immediate supervisor with the changes documented in the clinical notes. Performs selected skills, including the administration of treatment and medications, in the care of the sick, the maintenance of the health and prevention of illness of others, under the direction of the Professional Nurse. Prepares equipment and materials for treatment, observing aseptic techniques as required; meeting all OSHA standards. Assists the patient in learning appropriate self-care techniques and assessing patients with further needs for instructions. Assist Case Manager / DON with coordination of patient care within the Agency. Participates in regular conferences with RN, Case Manager, DON, and other health care staff to plan patient visits, progress and discharges. Attends nursing staff service meetings, in-services program, and required Agency meetings. Records and reports to the physician after conferring with the Case Manger / DON and other personnel any changes in the patient's condition of treatment. Has a full understanding and is in compliance with all Agency's regulations, policies and job descriptions. Identifies possible ineffective therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports to supervisor / DON. Advises and consults with the family to achieve the optimum potential of patients. Performs any related duty as designated by the RN, Case Manager, Nursing Supervisor or DON. Is supervised once a month by an RN Case Management, Field Supervisor, or DON. Any other job function given by the Administrator or Director of Nursing limited to state policies, regulations, and standards.
Qualifications:
Graduate of an accredited nursing program. Current state licensure as an LPN/LVN. Experience as an LPN/LVN commensurate with one of the following: One (1) year experience in an acute care setting within the last 24 months; Six (6) months home care experience within the last 12 months. Prior home healthcare experience is preferred. Valid state driver's license and reliable automobile. Proof of current automobile insurance. Current health certificate/physical examination and TB testing. Current CPR card. Flexible, yet organized, with the ability to exercise sound judgment.
Language Skills:
Able to communicate effectively in English, both verbally and in writing. Additional languages preferred.

Knowledge of general nursing practice.
