CenterWell Home Health, part of Humana Inc., is seeking a Home Health Licensed Practical Nurse to deliver compassionate in-home care to seniors and adults with chronic conditions. In this field-based role, you'll provide skilled nursing services, including medication administration, wound care, and patient education, while closely monitoring patient status and documenting in an EMR.
