Job Description
CenterWell Home Health, part of Humana Inc., is seeking a Home Health Licensed Practical Nurse to deliver compassionate in-home care to seniors and adults with chronic conditions. In this field-based role, you'll provide skilled nursing services, including medication administration, wound care, and patient education, while closely monitoring patient status and documenting in an EMR. You'll collaborate with an interdisciplinary team to support safe recovery, manage illness, and promote independence at home in a culture built on respect, teamwork, and professional growth. Responsibilities Provide in-home skilled nursing care to adult and senior patients per care plans Administer medications, treatments, and wound care following physician orders Monitor vital signs, document patient status, and report changes to the RN and physician Educate patients and families on disease management, medications, and home safety Collaborate with an interdisciplinary care team to coordinate services and update plans of care Ensure accurate, timely clinical documentation and compliance with regulatory standards Support safe transitions of care from hospital or facility to home Promote patient independence and adherence to therapeutic goals Required Skills Skilled nursing care in home settings Medication administration Wound care and dressing changes Patient and family education Care plan implementation Electronic medical record (EMR) documentation Vital signs monitoring and assessment Infection control practices Care coordination with interdisciplinary teams Time management and route planning
