The LPN is an essential member of the care team, providing direct clinical support, coordinating patient care, and ensuring accurate documentation while serving diverse populations, including underserved, LGBTQ+, and uninsured patients. Key Responsibilities Provide compassionate, patient-centered clinical support in a primary care setting Prepare patients for visits by obtaining medical histories, vital signs, screenings, and updating health records Assist providers with examinations, procedures, injections, phlebotomy, specimen collection, and point-of-care testing within scope of practice Coordinate patient care by managing referrals, diagnostic testing, medication requests, and follow-up activities Perform other duties within LPN scope of practice Travel to various locations as needed Qualifications Current Florida LPN license Minimum of 2 years of LPN experience preferred At least 1 year of phlebotomy and injection experience Current CPR certification Experience with electronic health records and clinical documentation Bilingual (English/Spanish) strongly preferred Valid Florida driver's license, proof of insurance, and reliable transportation Must pass Level II background check, fingerprinting, and E-Verify employment eligibility Metro Healthy Communities is an equal opportunity employer.
