Job Description

Overview We are seeking a passionate and dedicated Memory Support Director LPN to lead our memory care programs and ensure the highest quality of life for residents experiencing cognitive decline, including Alzheimer's disease and other forms of dementia. In this vital leadership role, you will oversee all aspects of memory support services, foster a compassionate environment, and coordinate interdisciplinary teams to deliver person-centered care. Your expertise will drive innovative care strategies, enhance operational efficiency, and promote a warm, engaging community for residents and their families. Duties Develop, implement, and oversee comprehensive care plans tailored to residents' unique cognitive and physical needs, ensuring adherence to best practices in dementia and memory care. Lead and supervise a team of caregiving staff, providing mentorship, training, and performance evaluations to foster professional growth and maintain high standards of resident care. Coordinate with nursing staff to ensure medication administration accuracy, monitor health status, and respond promptly to medical needs related to memory impairment. Manage daily operations within the memory support program, including scheduling, budgeting, resource allocation, and compliance with regulatory requirements. Cultivate strong relationships with residents' families by providing regular updates, education on dementia progression, and support for their involvement in care decisions. Promote an engaging environment through activities designed specifically for individuals with memory challenges that enhance cognitive function and emotional well-being. Ensure safety protocols are rigorously followed, including fall prevention strategies, emergency response procedures, and secure environment maintenance. Skills Must have a current LPN or above Licensure Extensive assisted living experience with a focus on senior care or memory support programs. Proven management skills with supervisory experience in nursing homes or group homes specializing in dementia or Alzheimer's care. Deep understanding of care plans tailored for residents with developmental disabilities or cognitive impairments. Strong knowledge of long-term care operations management, budgeting, and financial acumen necessary for effective program oversight. Expertise in medication administration protocols specific to seniors with complex health needs. Exceptional leadership abilities combined with excellent communication skills to motivate teams and engage families effectively. Familiarity with social work principles related to elder advocacy and resident-centered approaches in home & community care settings. $65,000.00 - $75,000.00 per year
