Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories. Administer medications, injections, IV infusions, and other treatments following established protocols and physician orders. Document all patient interactions accurately using EMR (Electronic Medical Records) systems such as Epic or Cerner, ensuring HIPAA compliance. Assist with complex procedures such as catheterization, suturing, and specimen collection/processing while maintaining aseptic techniques. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse needs including dementia, Alzheimer's disease, or acute pain management. Provide specialized care for patients in various settings such as long-term care facilities, assisted living communities, hospitals (including ICU and trauma centers), and home care environments. Support triage activities by evaluating patient symptoms and prioritizing urgent cases in emergency or urgent care settings. Work with individuals with developmental disabilities or behavioral health conditions to ensure safe and effective care delivery. Experience Valid LPN license with current state licensure required; additional certifications such as Basic Life Support (BLS) are essential. Prior experience working in medical-surgical units, emergency rooms, ICU, or level I trauma centers highly preferred. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks. Experience in diverse clinical environments including hospice care, pediatric or neonatal units (PICU), geriatrics, or post-acute care is advantageous. Knowledge of
ICD-9/ICD-10
coding standards and CPT coding for documentation accuracy. Skilled in patient assessment techniques involving physiology knowledge, anatomy understanding, and diagnostic evaluation processes. Familiarity with infection control practices, sterile processing techniques, and aseptic procedures is required to ensure safety standards are maintained at all times.
