In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient comfort, safety, and recovery by delivering skilled nursing services with enthusiasm and professionalism. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for acute pain or chronic conditions. Assist with diagnostic evaluations like laboratory specimen collection, phlebotomy, and medical imaging procedures. Support patients through procedures such as catheterization, tube feeding, wound suturing, and sterile dressing changes while maintaining aseptic techniques. Provide specialized care for individuals with complex needs such as dementia, Alzheimer's disease, memory care, or behavioral health conditions in settings like nursing homes or hospice care. Manage emergency situations including airway management, ventilator support, or acute trauma response in Level I Trauma Center or ICU environments. Educate patients and families on health coaching topics like medication administration, discharge planning, nutrition, and post-acute care instructions to promote optimal recovery. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification and registration. Proven experience in diverse healthcare environments such as hospital medicine, medical-surgical units, emergency rooms, or long-term care facilities. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation and patient records management. Strong knowledge of physiology, anatomy, ICD coding (ICD-9/10), CPT coding for procedures and treatments. Experience working with special populations including pediatrics, neonatal ventilator care, or individuals with developmental disabilities. Ability to perform patient assessments using critical thinking skills and provide appropriate interventions in urgent or emergent situations. Excellent communication skills to collaborate effectively with multidisciplinary teams and educate patients on their care plans.
