Responsibilities Deliver comprehensive nursing care to pediatric patients in their home environment, including medication administration, IV infusion, tube feeding, and catheter care. Conduct thorough patient assessments by monitoring vital signs, performing physical examinations, and evaluating physiological responses to treatment plans. Manage complex medical procedures such as ventilator support, sterile processing, and airway management while adhering to aseptic techniques and infection control protocols. Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Athenahealth, ensuring compliance with HIPAA regulations. Collaborate with families and caregivers to develop individualized care plans, provide health coaching, and educate on disease management including dementia care and Alzheimer's care when applicable. Triage patient symptoms over the phone or in person to determine urgency and appropriate intervention steps; coordinate with healthcare providers as needed. Assist with discharge planning, case management, and coordination of services for long-term or post-acute care patients across various settings such as assisted living facilities or nursing homes. Qualifications Valid LPN license with current state licensure to practice nursing. Proven experience in pediatric nursing, home care, or related settings such as hospital medicine or critical care environments like ICU or Level I Trauma Centers. Experience working with individuals with disabilities or developmental delays; familiarity with infectious disease care and pain management is highly desirable. Ability to perform complex procedures including venipuncture, phlebotomy, injection administration, sterile technique, and specimen collection/processing. Excellent patient assessment skills combined with physiology knowledge; capable of managing acute pain and respiratory support devices like ventilators. Demonstrated ability to work independently in a home setting while maintaining high standards of safety and documentation compliance.
