Holiday on-call only Role Overview This position is responsible for executing individualized patient care plans developed by RN/MD teams in a home health setting. The clinician will deliver hands-on skilled nursing care in patient homes while ensuring accurate, timely, andpliant clinical documentation. Key Responsibilities Execute care plans as directed by RN/MD including patient education and clinical interventions Perform skilled home health procedures such as wound care, catheter care, PICC line dressing changes, lab draws, andpression therapy Support hospital readmission prevention initiatives through proactive patient monitoring and education Maintain accurate, detailed, andpliant visit documentation for each patient encounter Ensure adnce to clinical guidelines, care standards, and regulatory requirements Coordinate with interdisciplinary care teams to ensure continuity of care Travel between patient homes within assigned coverage area (Fulton & DeKalb counties) Utilize home health documentation systems to record visits and clinical updates Maintainpliance with safety protocols and clinical best practices in a home setting Must-Have Qualifications Active State of Gia nursing licensure Minimum 1 year of home health experience Experience using HCHB (Homecare Homebase) software Strong clinical skills in home-based patient care delivery Preferred Qualifications Strong expertise in wound care management Experience with VAC therapy andplex wound cases Strong independent decision-making in home health settings Ability to manage a full patient load efficiently while maintaining documentation accuracy Strongmunication and patient education skills Additional Notes Navy scrubs preferred Must befortable working independently in a field-based care environment Strong documentation discipline and time management required Benefit offerings include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program,muter benefits and 401K plan.
