Aveanna Healthcare is seeking an in-home Licensed Practical Nurse to provide compassionate, skilled nursing care to pediatric and adult patients in their homes. In this role, you will deliver treatments, administer medications, perform wound, tracheostomy, and ventilator care, and closely monitor patient status. You'll collaborate with RNs, therapists, physicians, and families to follow individualized care plans while documenting accurately in electronic records. Responsibilities Provide in-home skilled nursing care to pediatric and adult patients per physician orders Administer medications, treatments, wound care, and tracheostomy/ventilator support as needed Monitor, document, and report patient status, vital signs, and changes in condition Collaborate with RNs, therapists, physicians, and families to support individualized care plans Educate patients and caregivers on disease management, medications, and equipment use Ensure compliance with Aveanna policies, clinical standards, and regulatory requirements Maintain accurate, timely clinical documentation in electronic health records Support a safe home environment and respond promptly to emergencies Participate in ongoing training, skills checks, and competency evaluations Uphold a compassionate, patient-first approach and professional communication Required Skills Medication administration Wound care management Tracheostomy care Ventilator management IV therapy and injections Electronic health record documentation Patient and family education Vital signs monitoring and assessment Care planning and coordination Infection control practices
