Area of Responsibilities
- Meets or exceeds productivity goals in accordance with Agency established 70th percentile.
- Perform thorough patient assessment each visit and keep Client Care Supervisor and Case Manager informed of any observations, changes, problems, complications, special needs and circumstances concerning care, the patient/family or home environment.
- Proficient in use of Maestro software system for documentation of patient visits and communication, entering orders, retrieving patient information, and maintaining a complete and accurate patient record.
- Verifies the Plan of Care prior to each visit and provides care according to physician's orders and Agency policies and procedures and implements appropriate nursing interventions to meet patient needs and change in conditions.
- Writes Verbal Orders/Change Orders for any changes to the Plan of Care (including PRN visits) on the day of the identified change and/or visit.
- Adheres to all Infection Control guidelines in accordance with Agency policy and procedures.
- Assists in the orientation of new staff members and/or student nurses as assigned.
- Attends all Case/Team Conferences regarding patients, as assigned, and provides input for care planning and reassessment.
- Completes any QA requests within 48 hours of receipt.
- Demonstrates maintenance of confidentiality of patient information.
- Ensures that clinical documentation evidences coordination of services with physician, other disciplines and other appropriate staff members.
- Ensures that patient home charts are kept accurate and current.
- Participation in in-services and/or continuing educational programs, staff meetings and case conferences is consistently demonstrated.
- Prepares appropriate documentation for mileage, visits and/or time and submits to the Client Care Supervisor on a timely basis.
- Provides for ongoing communication with appropriate staff members to make known any changes in patient needs, schedules, physician orders, special instruction and patient updates.
- Provides nursing care requiring specialized skills, per current nursing practice and Agency policies and procedures.
- Remains available to and is able to be located during all assigned working or 'on-call' times.
- Rotates 'on-call' as assigned for weekends and holidays.
- Uses supplies effectively and efficiently in a cost-effective manner and assures charges for any equipment and supplies for patient care are documented for appropriate reimbursement .
