Licensed Practical Nurse (LPN) - Pediatric Home Care (GAPP)
Conyers, GA Pay:
$30/hour
Schedule:
Approximately 30 hours per week Immediate Opening - Looking to Hire Very Soon! Modani Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for a pediatric client in the home through Georgia's GAPP (Georgia Pediatric Program). This client has a rare genetic disorder with developmental delays and requires ongoing skilled nursing services for routine assessments, seizure monitoring, medication administration, aspiration precautions, bowel management, and coordination of care to support continued health and safety. This position is ideal for an LPN who enjoys providing individualized care and building meaningful relationships with clients and families. Schedule Information This position is based on the client's Medicaid-authorized Skilled Nursing hours . 30 hours/week of Skilled Nursing have been requested and are currently pending authorization. The final work schedule will be determined by the client's approved authorization and care needs. Set or flexible hours may be available based on the approved schedule. While we strive to accommodate employee availability whenever possible, the client's schedule and care needs must remain the priority. Continued hours depend on the client's ongoing authorization and need for services. Additional pediatric assignments may become available as our program continues to grow but cannot be guaranteed. Responsibilities Perform skilled nursing assessments and monitor for changes in condition. Perform neurological assessments and monitor for changes in responsiveness, weakness, unusual movements, or changes from neurological/behavioral baseline. Assess respiratory status , particularly during periods of sleep/rest, and monitor for reported gasping, breathing changes, or signs of respiratory distress. Maintain aspiration precautions and monitor for coughing, choking, pocketing, fatigue, vomiting, breathing changes, or other signs of feeding/swallowing difficulty. Monitor bowel and bladder status , including incontinence, constipation, hydration, urinary changes, and signs of complications. Perform routine skin assessments , particularly with incontinence care, and monitor for redness, irritation, pressure areas, or skin breakdown. Assess mobility, balance, range of motion, brace fit, and fall/injury risk . Administer and monitor medications as ordered. Monitor medication effectiveness and potential adverse effects. Obtain and document vital signs as ordered and with changes in condition. Provide caregiver education regarding medications, aspiration precautions, respiratory concerns, bowel/bladder care, skin integrity, safety, and signs or symptoms requiring medical attention. Communicate significant changes in condition to the supervising RN, physician, and other appropriate members of the clinical team. Complete accurate and timely skilled nursing documentation. Qualifications Current Georgia LPN license in good standing Current CPR certification Valid Georgia driver's license Reliable transportation and current auto insurance Pediatric, home health, or GAPP experience is required Comfortable working independently in a home setting while collaborating with the clinical team Why Modani Care?
