Job Description

The Chanan Foundation is seeking a Licensed Practical Nurse (LPN) with strong organizational and communication skills to make a difference in the lives of the Special Needs community. The Chanan Foundation focuses on serving individuals with disabilities through housing, education, and life skills. Most importantly, we recognize and celebrate the ability of all people. The duties of this position will entail the administrative oversight of medical conditions for our Special Needs population under the supervision of the Registered Nurse (RN).
COVID-19
Vaccine is not required. The schedule for this position is Monday-Friday 8am-5pm, primarily working in an office setting. The LPN will work closely with the RN and other staff to provide oversight for clients, schedule medical appointments, and other duties as assigned. Essential duties will be as follows: Provide medical oversight and guidance for the special needs population under the supervision of company RN Work with doctor's offices for appointments/medication changes/medication refills. Track all annual doctor's appointments and schedule as needed. Track medication changes monthly and faxes to the pharmacy before monthly refills. Check-in monthly medication and verify accuracy against the MAR and consultation sheet. Track individuals annual Physicals, TB test, Vision, Dental, Mammogram, and OBGYN Update Health Risk Screening Tool Assist RN with employee trainings Assist RN with other duties as assigned Mandatory on-call rotation for after hour emergencies
