JOB SUMMARY
The LPN/LVN Skin Integrity Nurse is responsible for managing the skin program at the Georgia War Veteran's Home. This position involves evaluating residents' skin conditions, providing care and treatments as per physician orders, and maintaining compliance with professional standards of practice.
ESSENTIAL FUNCTIONS
Skin Assessment and Care:
  • Evaluate residents' skin conditions and provide care and related treatments in accordance with physician orders and professional standards of practice.
  • Measure skin areas and document findings in the resident's medical record.
  • Initiate and update care plans related to skin health.
Communication and Collaboration:
  • Communicate with physicians, residents, family members, charge nurse, and other interdisciplinary team members regarding resident status as appropriate.
  • Follow up with physicians to report resident skin impairments, transcribe and follow up with new orders.
  • Organize and conduct wound rounds with the center-specific wound consultant.
Documentation and Reporting:
  • Maintain up-to-date and accurate wound care reports, utilizing available computer programs.
  • Take responsibility for the prevention and healing of skin impairments.
  • Participate in regularly scheduled meetings to discuss residents with pressure injuries.
Quality Assurance:
  • Participate in Quality Assessment Performance Improvement (QAPI) activities as assigned.
  • Collaborate with the interdisciplinary team to assure accurate resident evaluation and development/revisions of individualized plans of care.
Professional Development:
  • Attend in-service education programs and participate in (Insert name of our online education training module) Training, as assigned, to learn new procedures and develop skills.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS
  • Works in office areas as well as throughout the facility and its premises.
  • Is subject to frequent interruptions.
  • Is involved with residents, family members, personnel, and visitors under all conditions/circumstances.
  • Works beyond normal working hours and on weekends and holidays when necessary. On call 24 hours a day, 7 days per week. Required Skills
  • Must have, as a minimum, one (1) year of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
  • Must have training in rehabilitative and restorative nursing practices.
  • Must possess a current unencumbered active license to practice as a Registered Nurse (RN) or LPN/LVN in the state of Georgia.
  • Must be able to read, write, speak and understand the English language.
  • Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
Required Experience
REQUIRED EDUCATION
  • Must possess, as a minimum, a nursing degree from an accredited college or university, or be a graduate of an approved Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) program.
