Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, hands-on nursing support across various clinical settings, including long-term care facilities, hospitals, outpatient clinics, and home health environments. Your skills will help ensure that patients receive safe, effective, and personalized care while supporting the overall mission of delivering exceptional health services. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusion and injection administration. Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing techniques to support patient treatment plans. Monitor patients' conditions closely by managing airway devices like ventilators and providing acute pain management for individuals with complex needs. Document all patient interactions thoroughly in medical records while adhering to HIPAA regulations to ensure confidentiality and compliance. Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute care services. Utilize critical thinking skills during triage assessments in urgent or emergency situations to prioritize patient needs effectively. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in diverse healthcare settings such as hospitals (including ICU or Level I Trauma Center), nursing homes, assisted living facilities, or home & community care environments. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for medical documentation and coding (ICD-9/10, CPT). Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques. Experience with specialized care areas including Alzheimer's/dementia/memory care, hospice care, infectious disease management, pediatric or neonatal care is a plus. Ability to perform venipuncture (phlebotomy), specimen collection/processing, and sterile techniques confidently. Excellent patient assessment skills combined with compassionate communication to support individuals with disabilities or complex health conditions.
