Job Description

Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients in a variety of settings, including patient homes and senior care facilities. This role involves working closely with registered nurses and physicians to ensure the highest quality of patient care. Duties Administer medications and treatments as prescribed by physicians. Assist in the development and implementation of patient care plans, including discharge planning. Maintain accurate medical documentation using Clinical Works or similar electronic health record systems. Provide education and support to patients and their families regarding health management and treatment plans. Collaborate with healthcare team members to ensure comprehensive patient care. Assist with the preparation, implementation, and continuing evaluation of the patient care plan. Recognize, and understand, the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients. Engage in continuous learning opportunities to enhance nursing skills and knowledge. Qualifications Valid Licensed Practical Nurse (LPN) license in the state of practice. Experience in a Home Health or teaching hospital setting is preferred but not required. Strong skills in medical documentation and familiarity with electronic health record systems such as eClinicalWorks. Excellent communication skills, both verbal and written, with the ability to teach patients effectively. Ability to lift patients safely and assist them with mobility as needed.
