Job Description Position Summary The Licensed Practical Nurse (LPN) provides direct nursing support, health monitoring, medication assistance, client education, and care coordination services across all organizational programs and service areas. The LPN works under the supervision of a Registered Nurse to support clients receiving IDD services, supportive services, housing assistance, personal care services, veteran services, and other community-based programs operated by the organization. The LPN assists in promoting client wellness, maintaining healthcare compliance, and supporting quality care delivery across multiple programs. Essential Duties and Responsibilities Direct Nursing Support Provide nursing care and health-related support services under the supervision of an RN. Monitor and document client health conditions, symptoms, and concerns. Assist with medication administration and medication monitoring in accordance with state regulations. Record vital signs, treatments, and healthcare observations. Assist clients with health education and wellness support. Care Coordination & Client Support Support healthcare appointments, referrals, and follow-up coordination. Communicate client concerns and changes in condition to supervising RN and appropriate staff. Assist with intake screenings and collection of health-related documentation. Collaborate with program staff to support client stability and wellness goals. Documentation & Compliance Maintain accurate and timely nursing documentation. Assist with maintaining compliance with healthcare standards, organizational policies, and licensing requirements. Participate in incident reporting and safety monitoring activities. Support infection prevention and health safety practices. Program & Operational Support Assist with nursing and wellness activities across organizational programs. Participate in team meetings, trainings, and interdisciplinary care coordination. Support outreach activities, wellness initiatives, and educational events as assigned. Perform additional duties necessary to support organizational operations and client care. Qualifications Minimum Qualifications Current and active Licensed Practical Nurse (LPN) license in the State of Georgia. Graduate of an accredited practical nursing program. Minimum of 1-2 years of nursing experience preferred. Current CPR certification required. Ability to work effectively with diverse populations and multidisciplinary teams. Strong communication, documentation, and organizational skills. Valid driver's license and reliable transportation may be required. Preferred Qualifications Experience in personal care homes, home health, supportive housing, behavioral health, or community-based services. Experience working with veterans, seniors, individuals with disabilities, or underserved populations. Familiarity with electronic documentation and case management systems. Knowledge of community healthcare resources and supportive services. Bilingual abilities are a plus. Knowledge, Skills, and Abilities Ability to provide compassionate and professional client care. Strong observation and reporting skills. Ability to maintain confidentiality and professionalism. Ability to prioritize tasks and adapt to changing program needs. Knowledge of infection control and medication administration practices. Commitment to client-centered and trauma-informed care.
