Job Description

## Job Overview We are seeking a dedicated Licensed Practical Nurse to join our healthcare team. The ideal candidate will have experience in home health setting with proficiency medical documentation,. ## Duties
  • Provide direct patient care in home health settings
  • Administer medications and treatments as prescribed by physicians
  • Assist with G-tube feeding and Trach Care
  • Perform medical documentation accurately and efficiently
  • Collaborate with the healthcare team to deliver high-quality patient care
  • Strong clinical skills with a focus on patient care
  • Ability to work effectively in a home health environment
  • Excellent communication and interpersonal skills
