Licensed Practical Nurse (LPN) - Pediatric Home Care (GAPP)
Warner Robins, GA Pay:
$30/hour
Schedule:
Approximately 40 hours per week Immediate Opening - Looking to Hire Within One Week! Modani Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for a pediatric client in the home through Georgia's GAPP (Georgia Pediatric Program). This client has a rare genetic disorder with developmental delays and requires ongoing skilled nursing services for routine assessments, seizure monitoring, medication administration, aspiration precautions, bowel management, and coordination of care to support continued health and safety. This position is ideal for an LPN who enjoys providing individualized care and building meaningful relationships with clients and families. Schedule Information This position is based on the client's Medicaid-authorized nursing hours. The current assignment offers 4 0 hours per week . The work schedule is determined by the client's approved schedule and care needs. While we strive to accommodate employee availability whenever possible, the client's schedule must remain the priority. As with all positions, continued hours depend on the client's ongoing authorization and need for services. Additional opportunities, such as supervisory visits or future pediatric assignments, may become available as our program continues to grow, but cannot be guaranteed. Responsibilities Perform comprehensive nursing assessments and monitor for changes in the client's overall condition. Monitor seizure activity, maintain seizure precautions, and administer rescue medication as ordered. Assess respiratory status, swallowing safety, and aspiration risk while reinforcing aspiration precautions. Monitor bowel function, skin integrity, and signs of infection or other complications. Administer prescribed medications and monitor for effectiveness and adverse reactions. Assess mobility, transfers, positioning, and implement fall prevention measures. Reinforce caregiver education regarding seizure management, medications, aspiration precautions, skin care, and safety. Communicate significant changes in condition to the supervising RN and physician. Coordinate care with physicians, therapists, and other members of the healthcare team. Complete accurate and timely documentation in accordance with GAPP and agency standards. Qualifications Current Georgia LPN license in good standing Current CPR certification Valid Georgia driver's license Reliable transportation and current auto insurance Pediatric, home health, or GAPP experience is required Comfortable working independently in a home setting while collaborating with the clinical team Why Modani Care?
