Job Description

Job Overview:
We are seeking a compassionate and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team in Winder. This role focuses primarily on completing initial client assessments and conducting routine supervisory visits for our home care clients. The ideal candidate is organized, confident working independently, and committed to maintaining high clinical and regulatory standards. This position offers flexibility and the opportunity to expand into additional clinical leadership or quality assurance responsibilities based on interest and experience.
Key Responsibilities:
Clinical Duties:
Conduct comprehensive initial in-home client assessments. Develop and update individualized care plans in accordance with Medicaid guidelines. Perform routine supervisory visits to ensure quality and compliance of services. Monitor client condition and report changes as appropriate. Provide clinical oversight and guidance to caregivers. Maintain accurate and timely documentation in the electronic medical record (EMR) system. Communicate effectively with case managers, families, and agency leadership. Ensure compliance with state regulations, Medicaid requirements, and agency policies.
Administrative & Compliance Duties:
Submit completed assessment and supervisory documentation within required timelines. Review caregiver documentation for accuracy and completeness. Assist with audit preparation and regulatory reviews as needed. Support care coordination efforts between the agency and external case management teams. Participate in quality improvement initiatives when applicable.
Skills & Qualifications:
Active Georgia RN or LPN license (required). Minimum 1 year of nursing experience (home care experience preferred). Knowledge of Medicaid waiver programs (preferred). Strong assessment and critical thinking skills. Excellent documentation and organizational abilities. Ability to work independently and manage a flexible schedule. Reliable transportation and valid driver's license. Strong communication skills and a patient-centered approach to care.
