Experience:
2 years of experience as an LPN is required.
Education:
Home Health, home care, hospice, and/or skilled nursing facility experience preferred. LPN license (HI) and BLS certifications required.

Contract Schedule:
Day Shift, 8:00am to 4:30 pm Kahu Malama Nurses is seeking an LVN/LPN - Home Health to join a growing and dynamic team!
Job Description:
Perform comprehensive patient assessments (vital signs, pain, intake/output, mental status) Monitor and report changes in patient condition to RN/MD Assist with care planning and goal setting with patients and families Conduct follow-up calls, discharge support, and patient education Coordinate care and services in home health settings Maintain accurate documentation, medication reconciliation, and specimen labeling Administer medications via oral, IM, SQ, NG/G-tube, eye/ear drops
Position Requirements:
