Join Our Team and Make a Difference in Patients' Lives! Greenbelt Home Care is seeking a compassionate, dependable, and motivated Licensed Practical Nurse to join our home health team. Position Summary:
The LPN provides skilled nursing care under the supervision of the RN Case Manager. This position works closely with patients, families, physicians, and the interdisciplinary care team to ensure individualized, compassionate, and effective care.
Responsibilities:
Provide skilled nursing care in accordance with physician orders and the established plan of care. Monitor and document patients' health status, reporting changes promptly to the RN Case Manager. Assist with medication management and administer treatments as prescribed. Perform wound care, catheter care, and other skilled nursing procedures. Educate patients and caregivers on disease management, medications, and home care techniques. Maintain accurate and timely clinical documentation. Follow all agency policies, state regulations, and Medicare guidelines. Participate in case conferences and communicate effectively with healthcare team.
Qualifications:
Current LPN license in good standing in the state of Iowa Current CPR certification Valid Driver's License, reliable transportation, and proof of auto insurance. Excellent communication and organizational skills. Ability to work independently while providing thorough, compassionate, patient-focused care.

$24.00 - $28.00 per hour
