GENERAL STATEMENT OF DUTIES
The Licensed Practical Nurse (LPN) provides high-quality, compassionate nursing care to patients managing chronic illnesses and end-of-life transitions. Working under the supervision of a Registered Nurse or Physician, the LPN balances the clinical requirements of primary care with the specialized symptom and pain management required in palliative care.
ESSENTIAL FUNCTIONS
Perform head-to-toe patient assessments and monitor vital signs, documenting changes in physical or psychological status. Traveling within a specified region to provide in-home services. Documents and codes all patient contact accurately and legibly in the medical record. Administer medications and treatments as prescribed, with a specific focus on pain relief, nausea control, and wound care for palliative patients. Educate patients and their families on care plans, disease progression, and the "why" behind specific treatments to empower informed decision-making. Collaborate with a multidisciplinary team (Physicians, RNs, Social Workers) to update individualized care plans and ensure seamless transitions between primary and palliative settings. Provide empathetic companionship and emotional stability to patients and families during traumatic or end-of-life situations. Maintain rigorous, accurate electronic health records (EHR) in compliance with HIPAA and state regulations. Ensure compliance with all medical regulations and protocols during home visits.

SPECIFICATIONS
Certification as a Medical Assistant (CMA or RMA) from an accredited program. Minimum of 1-2 years of clinical experience, preferably in a home healthcare setting.
QUALIFICATIONS
Current CPR/BLS certification. Active and unencumbered LPN license in Idaho Valid Drivers License Ability to work independently in various home environments. Must pass a criminal background check; This position is subject to pre-employment and random drug testing.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
Work is performed both in an office and a health services environment and requires sufficient mobility and physical reflexes to perform in both environments; Must be able to lift up to thirty-five (35) pounds unassisted; Local/regional travel required; may be required to travel for meetings, trainings, or to support other facilities; May be required to work irregular hours including nights, weekends, and on-call availability;
