$38.00 - $49.00 per hour (based on licensure, experience, and qualifications) Position Summary We are seeking a skilled and detail-oriented MDS Coordinator (LPN or RN) to join our team in Lewiston, Idaho. This role is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in compliance with federal and state regulations, while supporting interdisciplinary collaboration, care planning, and reimbursement accuracy. This is a key clinical leadership role within the organization, partnering closely with nursing, therapy, and administrative teams to ensure quality outcomes, regulatory compliance, and financial integrity. Key Responsibilities Complete and coordinate accurate and timely MDS assessments (RAI process) in accordance with CMS regulations Ensure proper documentation to support PDPM reimbursement and clinical integrity Collaborate with the interdisciplinary team (IDT) on care planning and resident assessments Monitor and support compliance with state and federal survey requirements Audit clinical documentation to ensure alignment with MDS coding and supporting evidence Serve as a resource to clinical staff regarding documentation and assessment best practices Participate in quality improvement initiatives and audits Qualifications Active LPN or RN license in the state of Idaho (required) Prior MDS/RAI experience in a skilled nursing facility strongly preferred Working knowledge of
PDPM, CMS
regulations, and care planning requirements Strong attention to detail and ability to manage multiple deadlines Effective communication and collaboration skills across departments Work Environment & Expectations Full-time, on-site position Typical schedule is Monday-Friday, with flexibility based on facility needs Fast-paced, collaborative skilled nursing environment High level of accountability tied to clinical quality and reimbursement accuracy
