Your Role:

Complete and maintain accurate MDS assessments and documentation Develop, review, and revise individualized resident care plans Ensure accurate documentation for reimbursement from all payor sources Coordinate with interdisciplinary team members to support quality resident care Monitor resident progress and update assessments as needed Support compliance with long-term care regulations and facility policies Help maximize resident outcomes and appropriate resource utilization Maintain accurate and timely documentation related to assessments and care planning Support resident dignity, safety, and well-being Perform additional duties as assigned by management in a resident-centered care environment

Who We're Looking For:

Current Illinois RN or LPN license required Long-term care experience required 1-2 years of MDS experience preferred Strong understanding of long-term care nursing practices and regulations Excellent communication, organizational, and documentation skills Dependable and professional team player with reliable attendance Passionate about providing compassionate care and improving resident outcomes Ready for a rewarding career?
