Current Openings:

Registered Nurse (RN) / Qualified Intellectual Disabilities Professional (QIDP/Case Manager)

Full-Time Schedule:

Monday-Friday | 8:00 AM-4:00 PM This unique dual-role combines nursing leadership with person-centered case management. You'll oversee healthcare services while helping develop and coordinate individualized supports that improve quality of life for the individuals we serve.

Responsibilities include:

Provide nursing oversight and clinical guidance Monitor health conditions and coordinate medical care Oversee medication administration and regulatory compliance Complete nursing assessments and maintain accurate documentation Develop, implement, and monitor Individual Service Plans (ISPs) Coordinate services with physicians, specialists, families, guardians, and community providers Participate in interdisciplinary team meetings Advocate for the health, safety, and personal goals of individuals served

Qualifications:

Current Illinois Registered Nurse (RN) license required Bachelor's degree preferred Meets or is eligible to meet Illinois QIDP requirements preferred Experience working with individuals with intellectual or developmental disabilities is preferred but not required Licensed Practical Nurse (LPN)

Part-Time or Full-Time Schedule:

Monday -

8:00AM - 4:00

PM or Monday-Thursday:

9:00 AM-3:00

PM, Friday:

9:00 AM-2:45 PM Responsibilities include: Assist with medication administration and health monitoring Maintain medical documentation and health records Coordinate routine healthcare needs Collaborate with the RN, Direct Support Professionals, and program staff Support a safe, healthy, and person-centered environment

Qualifications:
