  • Part-time, Full-time Overview We are seeking a dedicated and compassionate RN or LPN private duty nurse to provide personalized, in-home patient care.
The ideal candidate will possess extensive clinical experience across various specialties, including critical care, geriatrics, pediatrics, and post-acute care. This role offers the opportunity to deliver high-quality nursing services tailored to individual patient needs, ensuring comfort, safety, and optimal health outcomes. Candidates should be proficient in electronic health record systems such as Epic, Cerner, Athenahealth, or eClinicalWorks, and familiar with HIPAA regulations and medical documentation standards. Experience working with diverse populations—including those with developmental disabilities, memory care needs, or in hospice settings—is highly valued. Duties Provide comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations. Administer medications via injections, IV infusions, catheter care, and other routes following physician orders and safety protocols. Manage airway support including ventilator care and oxygen therapy as needed. Perform specialized procedures such as suturing, ostomy care, gastrostomy feedings, and catheterization. Monitor patient status through diagnostic evaluation and laboratory testing; document findings accurately in EMR/EHR systems. Coordinate discharge planning and collaborate with multidisciplinary teams for case management and post-acute care planning. Assist with mobility using Hoyer lifts or other assistive devices; provide personal hygiene and grooming assistance. Educate patients and families on health coaching, medication management, nutrition, and disease processes including infectious disease control and dementia care. Ensure aseptic techniques during all procedures to prevent infection; adhere strictly to HIPAA regulations to protect patient confidentiality. Respond promptly to urgent situations such as acute pain episodes or trauma incidents; manage airway emergencies effectively. Maintain all medical records meticulously including CPT/ICD coding for billing purposes. Qualifications Valid RN or LPN license in the state of practice with current certification in Basic Life Support (BLS). Proven experience in critical care environments such as ICU or trauma centers; hospital medicine; emergency room; or post-acute settings like hospice or home health. Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Experience administering injections, IV insertion/infusion therapy, phlebotomy, specimen collection/processing. Ability to perform complex procedures such as spinal taps or gastrostomy feedings is preferred. Excellent patient assessment skills with a compassionate approach to individualized care plans. Working knowledge of Workers' Compensation law is a plus for managing occupational health cases. Ability to work independently in a home setting while maintaining high standards of safety and professionalism.
