Job Description
We are seeking a compassionate and experienced Home Health Skilled Nurse (License Practical Nurse) to join our growing team. As a Visiting Nurse , you will provide one-on-one skilled nursing care to patients in their homes, helping them recover and maintain their health with dignity and comfort. Responsibilities
- Provide skilled nursing care in accordance with the patient's care plan
- Perform follow-up assessments, wound care, medication management, catheter care, and other skilled procedures
- Monitor patient health, progress, and communicate with physicians and care teams
- Educate patients and caregivers on disease management and treatment plans
- Maintain accurate and timely documentation using Axxess System
- Collaborate with interdisciplinary team for comprehensive care delivery
- Adhere to all agency, local, state, and federal regulations. Qualifications
- Current and valid LPN in the state of Illinois
- Minimum of [1] year of clinical experience (home health experience preferred)
- CPR Certification (BLS)
- Valid driver's license, reliable transportation, and auto insurance
