As a Visiting Nurse , you will provide one-on-one skilled nursing care to patients in their homes, helping them recover and maintain their health with dignity and comfort. Responsibilities
  • Provide skilled nursing care in accordance with the patient's care plan
  • Perform follow-up assessments, wound care, medication management, catheter care, and other skilled procedures
  • Monitor patient health, progress, and communicate with physicians and care teams
  • Educate patients and caregivers on disease management and treatment plans
  • Maintain accurate and timely documentation using Axxess System
  • Collaborate with interdisciplinary team for comprehensive care delivery
  • Adhere to all agency, local, state, and federal regulations. Qualifications
  • Current and valid LPN in the state of Illinois
  • Minimum of [1] year of clinical experience (home health experience preferred)
  • CPR Certification (BLS)
  • Valid driver's license, reliable transportation, and auto insurance
